Use this form to renew or change your existing Sabre Travel Network Internet Access Agreement.
1. Complete any changes to “Customer Information” and/or “Sponsor Agency” below, sign Agreement, and have Sponsor sign Agreement if applicable.
2. Send this Agreement as indicated below.
All changes will be effective within thirty (30) days of receipt of the completed, signed Agreement.

SYSTEM REQUIREMENTS: the following specifications or higher are recommended for optimum performance. Consult your Sponsor Agency for any other technical specifications.

Fill in the information for your new Sponsor in the “Sponsor Agency” section above and obtain signature in the Sponsor Signature section (if applicable).  By signing below, I authorize Sabre Inc. to remove Third Party Branch Access from the Sabre Pseudo City Code (my current Sponsor) effective with the acceptance of this Agreement.

Third Party Branch Access:
By checking the above, I authorize the following Sabre ConnectedSM agency (ies) to have access to my travel records, and release Sabre Inc. from any claim, loss, or damage caused by, or arising out of, the actions of the location(s) noted, including the use or misuse of the data contained in my travel records. Each third party branch access is subject to standard monthly branch access charges of $25.00 USD.

By signing below, Sponsor agrees to be responsible for all charges associated with this Internet Access to Sabre system license.

Once this form is received and verified, your welcome information, software (if applicable), and instructions will be sent to the e-mail address indicated above. Please allow 15 business days following receipt to receive order. All prices quoted are in US dollars. Keep a copy for your records and send a copy to your sponsor agency.

Agreement must be either (a) in writing and signed by authorized representatives of both parties; or (b) amended using an electronic form, including but not limited to, using “Click-On” technology and/or Electronic Signature technology.

For purposes of this Amendment, a “Click-On” amendment shall mean an agreement that Sabre posts electronically for review and consent by the Customer and which Customer either accepts or rejects by “clicking on” the appropriate button. “Electronic Signature” shall mean the secure, digital code attached to an electronically transmitted message the uniquely identifies and authenticates the Customer.
